The engineering trap of Chapter 4 does not originate in the individual engineer. It is produced by the organisation the engineer works inside, and it is produced reliably, because the organisation is scoring something other than examined reasoning.
Factory-style organisation is not broken. It is succeeding at the specification it was given: coordinate large numbers of people around a schedule, minimise internal friction, and orient attention toward whoever holds the approved answer. Every one of those is a legitimate objective. Together they select against the behaviour this volume needs.

What the standard objective selects for
Three behaviours are rewarded under approval-scoring, and all three are visible in ordinary engineering review.
- Source-tracking: attend to the person who can approve, rather than to the premise under discussion. Cheap, effective, and indistinguishable from diligence.
- Closure preference: terminate deliberation as soon as a candidate answer satisfies the room. Time-to-decision is measured; quality-of-reason is not.
- Falsifier suppression: never volunteer the condition that would put your own recommendation at risk, because doing so cannot raise your standing and can lower it.
The third is the one that matters for safety work. An organisation that has spent years penalising volunteered falsifiers cannot then ask its engineers for unprompted falsifiers on a launch review and expect to receive them.

Six procedures
The following are auditable, cheap, and reversible. Each changes what the organisation scores rather than exhorting people to think better.
- 1. Status labels on claims. Every substantive claim in a design document carries one of: measured, derived, reported, or stipulated. Reviewers may challenge the label independently of the claim.
- 2. A named falsifier and a review date. Each architectural decision records the observation that would make it wrong, and the date on which that observation will be checked.
- 3. Commit before consult. Participants record their own position before the senior view is stated. This preserves the independent signal that the meeting would otherwise destroy in its first two minutes.
- 4. Reasons for revision. A changed position is recorded with the reason for the change. A revision with no recorded reason is logged as an unjustified revision — the organisational analogue of the U statistic.
- 5. Withheld answers in review. A reviewer who already holds the answer declines to supply it and instead selects the stressor most likely to force a reconstruction. This is the discipline of the guide, and it is the expensive one.
- 6. A decision ledger. Append-only. Records the claim, its status, the stressor applied, the reason recorded, and the falsifier. Never rewritten, only appended to.
The ledger is what makes the rest measurable. Without it, falsifier production and unjustified-revision rate cannot be computed, and the organisation is left with impressions about its own reasoning quality.
The cost, stated plainly
Procedure 5 is the load-bearing one and the one most likely to be dropped. Withholding an answer you already hold is slower than supplying it, and the senior engineer who withholds looks less useful in the short run than the one who resolves the question immediately.
That cost is real and is not argued away here. The claim is only this: an organisation that never pays it cannot distinguish an engineer who understands the system from one who has learned what the review expects to hear — and neither can its safety process.
Status note
Design proposal. The six procedures are auditable and cheap to run, and none of them has been shown to improve performance, profit, resilience, retention, or safety outcomes. No such claim is made here. The organisational objection stands unresolved: a practice that depends on sustained senior attention may be distributed unequally, and could widen the very asymmetry it hopes to reduce. Falsifier: if teams running all six procedures show no change in falsifier production or unjustified-revision rate against matched teams, the procedures are ceremony.
